Along with your completed paperwork, please bring the following with you to your scheduled appointment:

• Bring films/disc and reports on any recent MRI, CT scan, or X-rays.
• Co-payments
• Insurance card(s)
• Photo ID
• Referrals (if required)

For MRI scans, wear loose-fitting clothing (no metals).
